• List established recommendations (as per the International Council for Standardization in Hematology (ICSH) consensus documents of 2018 and 2021) for monitoring or assessing direct oral anticoagulant (DOAC) plasma levels?
• Summarize differences between warfarin and DOACs as anticoagulants, and why DOACs are used increasingly.
• Identify clinical and laboratory features that may lead to requests for laboratory monitoring of DOAC plasma levels in addition to those recommended by the ICSH.
